Sometimes it is given when you are really close to leveling up. Then when you lol up, you don't get it as it was given earlier. Kinda buggy.

There use to be a bug where if you leveled up off an opponent that quit the match you wouldn't get the reward. The best way to avoid it was to level up on single player as they can't quit the match :)
